Heart Health & Wellness - applications due 4/12

Reclaim Our Hearts: Reclaim Our Hearts is a free 15-month heart health wellness program designed to support women in Dane County, living with clinically diagnosed high blood pressure (hypertension). Through monthly heart health circles, wellness activities and classes, walking groups and optional support from a Community Health Worker, participants are empowered to improve their heart health, regulate their stress and move with intention.
